If you are a first-time client, you should receive an online intake to fill out via e-mail. Alternately, you may print this form, complete it, and bring it with you to our first appointment. If you have the ability, print 2-sided to save paper!
New Health (Cosmetic) Surgery Detail
If you are coming for post-surgery lymphatic drainage, please print and complete this form IN ADDITION to the General, Online Massage Intake.
General Liability Release Form
This is a standard release of liability for our massage therapy work together.
This form allows a parent or legal guardian to give permission for me to perform massage on a minor client. Please print and bring or send with your minor child.
